Your application will need to be completed prior to an interview being scheduled. Please bring the below items with you to your in-person interview:

• Current professional license

• CPR/BLS certification card

• Copy of a current TB test within the last year (or current chest x-ray and positive symptoms review form)

• ACLS, PALS, or any other certificates related to your job

• Photo ID

• Driver's License

• Documents that establish both identity and employment authorization (i.e. Social Security Card, Birth Certificate and/or Passport)

• If you are not a resident of the United States, we will need your Permanent Resident Card

• Copy of Immunization Record with proof of MMR, Td/TDAP, HBV (Hep B), Varicella (Chicken Pox)

• Current resume

Please be aware that in the event you wish to be employed by a facility that we send you to, that facility is obligated to pay NPH a finder's fee. This is an arrangement between the facility and our company that is necessary for you to acknowledge.
